When determining a course of treatment, it's important to consider the cause of teeth discoloration, the condition of the teeth and gums, and the patient's general medical history," Nadia Kiderman said. Different discolorations beget different whitening treatments, Dr. Kiderman notes. Yellowish teeth are typically the best suited for bleaching, which can be accomplished via whitening toothpastes and rinses, over-the-counter gels and strips, tray-based whiteners, and in-office techniques. Alternatively, brown- and blue-gray stained teeth (caused by tetracycline) For these extreme cases, Dr. Kiderman recommends alternative whitening techniques: dental veneers, bonding, and crowns. As these methods involve covering up stained teeth, not lightening them, they work regardless of the degree of staining. Alternative whitening techniques are also ideal for patients with preexisting dental restorations; Patients with yellowed teeth in search of traditional bleaching methods have a wide variety of options to choose from, Dr. Kiderman advises. Whitening toothpastes and rinses are perhaps the most simple whitening systems. Available over-the-counter, these techniques contain carbamide peroxide or hydrogen peroxide — the same chemicals dentists use, but in lesser quantities. Whitening toothpaste can usually whiten teeth by one shade, while whitening mouthwash is considered less effective because the chemicals are only in contact with teeth for a couple of minutes each day, Kiderman notes. Peroxide-based whitening strips and gels can also be effective; they generally take several days to take effect and last for three to four months. Tray-based whitening systems employ a similar method but facilitate extended contact with teeth — from one or two hours to overnight. Whitening trays can be purchased over-the-counter or specially fitted and monitored by a dental professional. "Over-the-counter methods can be extremely successful, but for the fastest results, professional treatment is the way to go. In one 30- or 60-minute session, patients can see results, and after several sessions dramatic results can be observed," Dr. Kiderman said.
