AZ’s Largest Independent Physician Organization Creates Integrated Network to Support Patient CareThe Arizona State Physicians Association sees independent physicians as key leaders in the movement towards value-based care
“Over one million patients in Arizona are eligible to receive care from an ASPA provider,” said Connie Richardson, executive director of ASPA. “Our goal in creating ASPA-Connected is to enable greater patient access to the state’s largest network of independent providers and support our physicians to better coordinate on patient care. Through ASPA-Connected, we will focus on building the network around collaborative, value-driven contracting with payers to support our patients more coordinated care, and drive toward better quality and lower overall health costs.” Achieving the triple aim with a patient-centered approach As government and commercial payers shift to value-based reimbursement, providers must navigate requirements to achieve the triple aim. The triple aim is a widely used framework to demonstrate improvements in health outcomes and patient experience, while reducing costs. ASPA-Connected has consulted with Ideal Health IT to implement the services and support needed to achieve this goal. As a result, Practice iQ, a Healthagen business, will deliver embedded care management, integrated technology, and business services to providers in ASPA-Connected. This collaborative effort will allow ASPA-Connected providers to have a more complete view of their patient population and be able to better engage them through technology and care management. For at-risk patients, this means delivering programs to help prevent avoidable health issues, managing existing health issues, and minimizing the progression of chronic disease. All patients in the ASPA-Connected network should experience a more coordinated, engaging and personalized experience as a result of the patient-centered approach. Independent providers leading value-based care movement “ASPA believes the independent provider is best positioned to lead the movement towards value-based care. We created ASPA-Connected as the first step to accomplish this goal, and we look forward to growing the network collaboration with more providers, payers and patients in the near future. In my 22 years leading the organization, I have never been more encouraged for our affiliated providers and the patients they touch,” Richardson said. --- About Arizona State Physicians Association (ASPA) ASPA is an Independent Physicians Association with more than 2,400 participating providers representing all major physician specialties as well as physician therapists, psychologists and other healthcare providers.
