Community Care to receive prestigious Wisconsin Forward Award December 10 in Madison.

BROOKFIELD, Wis. - Dec. 10, 2015 - PRLog -- Brookfield-based Community Care, Inc. is one of three Wisconsin organizations to receive a 2015 Wisconsin Forward Award, considered the premier recognition for workplace excellence.

Monona Terrace Community and Convention Center in Madison and the Columbus Community Hospital in Columbus will also be honored tonight during an awards ceremony and gala at the Monona Terrace in Madison.

Community Care, a nonprofit long-term care organization serving more than 10,000 vulnerable Wisconsinites, earned the Challenger Award for the development of its organizational profile.

The Wisconsin Forward Award Program is designed to recognize Wisconsin organizations that strive to be role models. Modeled after the Malcolm Baldrige National Quality Award, WFA's evaluation process is based on the Baldrige Criteria for Performance Excellence, which provides a framework and criteria to guide organizational performance. Organizations submit a self-assessment to the program for an in-depth review and written feedback from a team of volunteer Examiners.
