What is New York's NHTD Medicaid Waiver program?

 
BROOKLYN, N.Y. - Oct. 12, 2022 - PRLog -- The New York State, through several programs, endeavors to extend equal rights to all New Yorkers, regardless of their age and health status. New York's Nursing Home Transition and Diversion Waiver (NHTD) Medicaid Waiver Program assists senior citizens and individuals with physical disabilities access and receive comprehensive services while living in a community-based setting as compared to living in a nursing home, congregate care setting, or other institutions.

What Are the NHTD Eligibility Requirements?

Once you or your loved one become a NHTD waiver program participant, you receive a personalized, Initial Service Plan (ISP) that ensures your health, independence, community integration through state-funded housing, and welfare. It's worth noting that the NHTD participants are primary decision makers – they work with caregivers and NHTD providers to develop their ISPs.

To qualify for New York's NHTD waiver program, you must:
  • Be between 18-64 years old and have proof of physical disability
  • If 65 years and older - meet the requirements for nursing home level care.
  • Be eligible for a Community-based Medicaid.
  • Be in need of waiver services to ensure health and welfare in the community.
  • Prefer community living to living in a home or nursing facility.
  • Be able to live safely in the community with the services and support extended by the NHTD waiver program, Medicaid, and other community programs.

How Does One Become an NHTD Waiver Program Participant?

All referrals must begin with the call to RRDC (Regional Resource Development Center).
If the applicant is currently residing in a nursing home or health care facility, the discharge planner, social worker, or Open Doors Transition Specialist supporting the individual may call or write the RRDC.
During this referral phone call, RRDC staff will confirm the applicant meets the basic eligibility criteria and collect information needed to complete a referral form. On this call, the applicant will also provide consent for the referral.

The RRDC will follow up by contacting the applicant to arrange a meeting and the referral moves to intake or the appointment can be arranged during the call.
