One thing that causes them is bad soil quality. Soil quality is in charge of a number of different functions for the tree, and without the right soil a tree can begin to become unhealthy/not grow as much as it should. When in the right soil, it's not common for roots to become exposed unless the soil itself is powdered and loose.
