The Boston Scientific Foundation grants $30,000 to Tri-City Health Center for the HOPE Project.

Tri-City Health Center (TCHC) has been awarded a $30,000 grant by The Boston Scientific Foundation for the Homeless Outreach for People Empowerment (HOPE) project providing health care and social services to the homeless population in Alameda County.
 
June 21, 2010 - PRLog -- Fremont, CA - Tri-City Health Center (TCHC) has been awarded a $30,000 grant by The Boston Scientific Foundation for the Homeless Outreach for People Empowerment (HOPE) project.  The HOPE project operates a state-of-the-art mobile health clinic in collaboration with TCHC as the medical service provider to bring "one-stop" health care and social services to homeless individuals and families residing in shelters,transitional and impermanent housing situations. The funds provided by The Boston Scientific Foundation will enable the HOPE project to more vigorously and confidently administer needed primary care and behavioral health services to TCHC's homeless and most vulnerable populations.


The $30,000 grant from The Boston Scientific Foundation supports the HOPE Nurse Practitioner's time caring for patients and provides necessary equipment and supplies for the project. Between May 1, 2010 and April 30, 2011, TCHC will use the grant funds to provide medical services to 500 individuals, refer 100 homeless patients for health insurance assessment and review, provide winter relief to 25 patients, provide 298 transportation vouchers, and refer all patients without lab tests to TCHC’s lab.

The HOPE Project, now in its 10th year of service, is supported by multiple partners and includes: Tri-City Health Center (medical care), Abode Services (homeless housing and social services), the Cities of Fremont and Livermore, California (fiscal agents) and local churches and community groups (site locations, meals, supplies). In collaboration with these partners, TCHC is committed to providing critical services to the homeless population.
