The TCHC Community Awards and Breakfast Fundraiser will recognize patients that set positive community examples by following their healthcare plan to improve their overall health condition. Assemblymember Bob Wieckowski will also be recognized with an award for his continuous commitment to public health care. Proceeds from this event will go towards improving patient care and service delivery. TCHC services have expanded to include medical, dental, and behavioral services that meet the needs of individuals and the entire family. Over the years, TCHC has stayed true to the mission of providing quality health care services to the underserved and uninsured families in communities of Fremont, Newark and Union City, without regard to financial position, ethnicity, language, culture, sexual orientation, or the ability to pay. One of the bright spots in America’ · Reducing economic and ethnic health disparities nationwide, even in the poorest and most challenged communities. · Producing significant cost savings for our health system. · Reducing unnecessary hospitalizations and unnecessary visits to the emergency room. · Generating a system of preventive medicine that patients use regularly to stay healthy, thus saving lives. Health centers serve more than 22 million people nationwide, and are increasingly becoming the trusted provider of choice for many families, whether they are insured or not. Evidence shows patients choose health centers because they are convenient, affordable, and offer a range of services from a team of caring professionals. Studies also show the quality of care at a health center is as good as or even better than private practices. “Every day in our waiting rooms I witness the value of having a patient-centered health home,” The Community Awards and Breakfast Fundraiser event will be held at the Fremont Marriott Hotel located at 46100 Landing Parkway, Fremont, CA 94538. The breakfast will start at 7:00am followed by the awards ceremony. Tickets are $75.00 including breakfast. End
