By: The Dental Hub Para molar is a developmental anomaly and has been argued to arise from a combination of genetic and environmental factors. This article of Dr Prakruti Trivedi presents a case report of an unusual occurrence of a paramolar in the maxilla in an otherwise healthy individual. Supernumerary teeth or hyperdontia, is an odontostomatologic anomaly and may be defined as any teeth or tooth substance in excess of usual configuration of 20 deciduous and 32 permanent teeth. Supernumerary teeth may occur singly, multiply, unilaterally or bilaterally, erupted or impacted, in one or both jaws, in deciduous as well as permanent dentition. Supernumerary teeth are more frequent in males than in females. Asian population are more affected than others. Supernumerary teeth are found more in maxilla than in mandible. Case Report A 28 years old male patient came to Dr Prakruti Trivedi at The Dental Hub with a chief complaint of pain in upper left back region of mouth since last 3-5 days. Patient's medical and family history was non contributory and there was no sign of any systemic diseases or syndromes. Intra-oral Examination Intraoral examination done at The Dental Hub revealed a normal class 1 dentition with well aligned upper and lower arch. In addition to full completion of permanent teeth there was an extra tooth present buccally between 1st and 2nd maxillary molars. This tooth was diagnosed by Dr Prakruti Trivedi as a paramolar. The crown of this paramolar had two cusps and quite resembled a permanent premolar. Clinical Examination On clinical examination, done by Dr Prakruti Trivedi at The Dental Hub, caries was detected on palatal surface of paramolar. Following clinical examination, periapical radiograph was taken. It revealed paramolar is single rooted tooth. Patient was informed of existing condition and extraction of paramolar was advised as it was carious and maintenance of oral hygiene in this area was difficult and there was possibility of food lodgement, recurrence of dental caries, and detoriation of the surrounding periodontal health. The tooth was extracted at The Dental Hub by Dr Prakruti Trivedi under local anaesthesia. The extracted tooth had a normal morphology. The length of the root was normal relative to its crown. The root apex was completely developed. Discussion The occurence of paramolar is relatively uncommon.
