Scottsdale, Arizona -  January 1^st, 2016  --  The National Readmission Prevention Collaborative (NRPC) announced today that ComForCare –3 Valley offices  has successfully completed its certification program and has earned the rare distinction of Nationally Certified Readmission Prevention (NCRP) Partner.  ComForCare of Arizona  with its 3 offices serving the entire Valley of  Phoenix, Scottsdale and Sun City  is one of the first healthcare organizations in the state to receive this prestigious honor, held in high regard by leading acute hospitals how are now being penalized financially if patients are admitted unnecessarily to the hospital.
