DALLAS - March 8, 2022 - PRLog -- PARKLAND COMMUNITY HEALTH PLAN NOTICE OF DATA BREACH

DALLAS (Mar. 8, 2022) – Parkland Community Health Plan (PCHP) recently discovered an incident involving the personal information of a subset of its members. On or about Jan. 4, 2022, PCHP discovered that an I.D. card mailing intended for 1,682 of its members was mailed to other PCHP members in error due to a vendor printing issue.

Some of the following information may have been viewed by another PCHP member as a result of the mis-mailed information: name, PCHP ID number, provider information and plan/copay information. This information did not include social security number, driver's license number or any financial account information.

PCHP began an investigation immediately upon discovery of the printing issue and has taken steps to address the matter with its print vendor. While we do not believe there has been a misuse of any personal information, all affected members have been notified in an abundance of caution. New I.D. cards have been sent to the correct members, and we have sent individual notices to members whose personal information was impacted by the event.

PCHP is committed to protecting its members' personal information and we take our responsibility very seriously.
