PRLog -- ChartSpan, a leading provider of Chronic Care Management (CCM) solutions, and Main Line Health — a health system delivering patient-centered care to residents of Philadelphia and its suburbs — are now collaborating to bring Chronic Care Management services to thousands of Medicare beneficiaries.
Chronic Care Management ensures that Medicare patients with two or more chronic conditions receive preventative care each month. CCM reduces patients' risk of hospital readmission and ER visits, leading to better overall health outcomes.
The collaboration between ChartSpan and Main Line will emphasize:
- Increased Patient Engagement: Enrolled patients will have access to a 24/7 phone and text line for after-hours care and will receive monthly calls from a designated care manager. During these calls, they can ask questions, share health updates, and benefit from assistance with appointment scheduling, health goals, and medication management.
- Personalized Care: Patients will receive personalized care plans, based on recommendations from their providers and the patients' own preferences. These care plans will reflect their medical history, desired outcomes, values and goals, empowering the patient to take control of their own health.
- Greater Healthcare Access: Chronic Care Management will expand access to care for Medicare patients throughout southeastern Pennsylvania. In addition to remote, monthly check-ins, CCM also includes Social Determinants of Health assessments and directions to local community resources.
