By: Garcia & Artigliere Admitted to the psychiatric care facility as a dependent adult on August 26, 2019, the plaintiff arrived with a medical history that included partial epilepsy, hallucinations, anxiety disorder, suicidal ideation, and borderline personality disorder. Because of these serious mental health conditions, she was placed on a 15-minute check rotation for self-harm and other dangerous behaviors. The day after she was admitted, the plaintiff's physician also ordered constant 1:1 supervision due to risk of suicide. The staff at Del Amo Hospital had a clear responsibility to provide the plaintiff with constant supervision, and yet she was routinely left unsupervised, resulting in at least two major incidents of self-harm. Within only three days of her admission to the facility, the plaintiff was discovered dead as a direct result of this negligence.
